Description (Service) Culturally sensitive homemaking and assisted living care services for older adults with age related health conditions and/or disabilities who self-identify as First Nations, Métis or Inuit

Homemaking Supports
  • light housekeeping
  • laundry
  • sweeping/mopping and dusting
  • personal hygiene care
  • medication support
  • meal preparation
  • social visiting
  • telephone reassurance
Transportation
  • local trips to shopping centres/banking, in Barrie only
Dementia supports:
  • education to increase awareness to support earlier identification and reduce stigma of Dementia
  • increase community support services to Indigenous families managing Dementia
Program is a partnership between Helping Hands and Barrie Area Native Advisory Circle

Eligibility For Homemaking Support
  • Individual lives in Barrie or Orillia
  • Must self-identify as First Nation, Metis or Inuit
  • Must be an older adult (55+) and/or an older adult with age related health conditions or disabilities

For Transportation Services
  • individual must live in Barrie, local transportation only
  • Must self-identify as First Nation, Metis or Inuit
  • Must be an older adult (55+) and/or an older adult with age related health conditions or disabilities

For Dementia Support
  • Individual lives within North Simcoe Muskoka Home and Community Care Services area
  • Must self-identify as First Nation, Metis or Inuit
  • Must be an older adult (55+) and/or an older adult with age related health conditions or disabilities
Application
  • Call Indigenous Care Coordinator at 705-627-3499 or email icc@banac.on.ca
  • Referral form can be filed out through self-referral, partnering agency, community organization or from a family physician
  • Form can then be directly submitted to the Indigenous Care Coordinator to confirm the eligibility
  • Indigenous Care Coordinator will personally meet with the individual that is new to the program, to fill out the intake and assessment forms to identify any needs or requirements for the services provided, to develop a plan of care and to bridge the new connection between client and IILC worker
