Description (Service) Expert nurses providing care through a teaching model, services include:
* wound care
* IV medication administration and teaching
* PICC lines, Portacath, Chemo disconnects
* ABIs
* Health Teaching
* Injections
* Symptom management
* Ostomy care and teaching
* Catheter care, Nephrostomy care, Drain care, G Tube care
* Physical Assessments

All patients attending the community nursing clinic will be assessed and provided with individualized instruction on self care and self-management supports

Hours Clinic: Mon-Sun 9 am-9 pm
Areas Served Grey County - eastern part ; Muskoka District ; Simcoe County - south to Cookstown
Boundaries All of District of Muskoka * most of Simcoe County (south to just south of Hwy 89) * small portion of eastern Grey County, including Maxwell and Feversham
Eligibility North Simcoe Muskoka Local Health Integration Network (NSM LHIN) Home and Community Care patients
Application By referral only * patient will be contacted by St Elizabeth with appointment details * patients can schedule an appointment at times that is convenient for them
