Description (Service)

Assessment and intervention services for infants and preschool children who are blind or low vision as diagnosed by an ophthalmologist

Child and Family Intervention Services and Childcare Consultation available
provides a Family Support Worker to families of eligible children where indicated
will assist families to access service when low vision/blindness is suspected but accessing a diagnosis is difficult

Eligibility	Age Requirements: 4 year(s) and under Screening - infants children up to junior kindergarten age diagnosis blind or low vision
Application	Referral - Families Referral - Physicians Referral - Optometrists Referral - Early Interventionists (with written diagnosis from an ophthalmologist) Intake worker gathers basic information and mails a background information package for child's family to fill out
