Description (Service) Clinic providing quality primary health care services for patient care, providing health promotion and disease prevention * Registered Nurse, Clinical Pharmacist, Social Worker, Occupational Therapist, and Nurse Practitioner offering:  
 
* diabetes program, including a diabetic foot care program 
* hypertension program assisting patients with blood pressure measurement, self care and management education 
* medication check, smoking cessation  
* patient counselling in addiction and mental health, domestic violence, support for the elderly and general counselling.  
* fall prevention assessments, in-home and general occupational therapy assessments 
* FAST clinic operated by Nurse Practitioner as an overflow for the local Hospital Emergency Department 
* Team Unbreakable - a learn how to run program for ages 14-18 years * an engergizing approach to feeling less stressed, worried, and down

Hours Office: Mon-Thu 8:30 am-4:30 pm * Fri 8:30 am-12 noon * program times vary

Eligibility Programs are for patients of FHT through self, community or physician referral (with the exception of FAST clinic) 
 
FAST clinic (nurse practitioner) - available to anyone through their physician's referral
