Service: Vulnerable Persons Registry

Description (Service) Voluntary registry for vulnerable people and their family members or caregivers to submit essential personal information that may be used by responding officers during an emergency situation 
Serves people with a condition that puts them at increased risk, including:  
* a medical, mental health or physical condition 
* a cognitive, physical, intellectual or developmental disability 
Risk factors may include a tendency to wander, the inability to communicate, unusual social responses and fascinations or attractions to dangerous locations 
Information required for registration includes: 
* name, address, contact information 
* date of birth 
* recent photograph of the individual (optional) 
* physician's name 
* medical condition, special needs 
* de-escalation techniques 
* tendencies/routines 
* objects of attraction 
Registry information is updated annually
