| Description (Service) | Provides temporary health services to patients who DO NOT have a family doctor or nurse practitioner, while they wait to be connected to a local family physician. Patients at this clinic must be residents of Collingwood, Clearview Township, Wasaga Beach or have a L9Y Blue Mountain postal code. All Shoreline Health Clinic patients are automatically added to a local waitlist for a family doctor and will be placed with a permanent provider as soon as one becomes available. Once a permanent provider becomes available, the patient will no longer have access to care from the Shoreline Health Clinic. Services at this clinic are covered by OHIP and are provided at no cost. Due to the limited number of appointments available at this clinic, concerns that require extensive follow-up will not be seen. To register for this clinic you must complete an intake form. Please follow the process outlined in the application section. |

| Eligibility | Patients who live in Collingwood, Clearview, Wasaga Beach and those in The Blue Mountains with L9Y postal code and do not have access to a family doctor or nurse practitioner anywhere in Ontario. Patients with a physician anywhere in Ontario are unable to access service at the Shoreline Health Clinic. Note: Anyone who already has a physician in Ontario, but is looking for a local physician can complete the Primary Care Waitlist form. Primary Care Waitlist form. |
