Description
Description (Service) Non-emergency, community-based service supports vulnerable patients living independently in the community. Aims to reduce 911 calls, emergency department visits and hospital admissions. Services include: 
     
  • in-home assessments 
  • safe home assessments 
  • scheduled wellness checks 
  • remote patient monitoring 
  • direct communication with primary care providers 
  • diagnostic tests 
Service Details
Hours Community Paramedics are available 24 hours a day/7 days per week 
Mon-Sun 7 am-10 pm (in-person appointments) 
Mon-Sun 10 pm-7 am (virtual only)
Areas Served Dufferin County
Eligibility Patients who require additional assistance while living in their home, including high-risk clients living with complex medical issues such as diabetes, COPD and CHF

Referrals from patients, family members or service providers will be reviewed upon receipt to determine eligibility for program.  
Community Paramedics will make initial contact with the patient within 24 hours after receiving a referral to discuss outcome.
