Description (Service)

Adult Protective Service Workers (APSWs) support individuals 18 years and older who have a developmental disability who live or are planning to live independently in their community. APSWs assist people with planning, coordination and case management of community resources, advocacy, facilitation of community access and inclusion, monitoring of supports, resource information and community development. APSW support is voluntary and focuses on the individual's needs, strengths and goals.

Hours Office: Mon-Fri 9 am-5 pm
Areas Served Huron County
Eligibility Individual (over age 18) must have a developmental disability and live, or plan to live, independently in Perth County.
