Description (Service) | COVID19 Not taking new referrals. Assists people and families with diabetes to learn about living well with diabetes and how to deal with the day to day challenges * Registered Dietitian and Registered Nurse, Certified Diabetes Educators see both inpatients and outpatients, and work with their doctor to decide best care plan * services are for client with Type 1, Type 2, Gestational and Prediabetes clients, services include: * individual and group education * nutrition counselling, menu planning, virtual grocery tours, reading labels * assessment, clinical monitoring and management * provision of blood sugar meters and education on their use * focus on prevention of complications and development of healthy lifestyle * ADP Certified Insulin Pump Therapy Program |
