Description (Service) Provides primary care for individuals who either lack primary care or require more comprehensive primary care than they are currently able to receive from their family physician or nurse practitioner.

Available to individuals with a serious mental illness and/or addiction issue for which they are receiving local mental health and/or addiction services.

The Nurse Practitioner and Registered Nurse can provide a variety of services, including, but not limited to:
  • comprehensive health assessment
  • monitoring and management of chronic diseases such as diabetes, arthritis, asthma and heart disease
  • prescribing and monitoring of medication
  • immunizations
  • health teaching
  • referral to specialists and other human services
  • consultation and service coordination.
The program is delivered in collaboration with the Brockton and Area Family Health Team and four physician groups. It is based out of Walkerton and Owen Sound with outreach to Durham, Hanover, Chesley, Paisley, Kincardine, Southampton, Wiarton, Port Elgin.
