| Description (Service) | Works to link and refer people to appropriate services where available by providing assessment and support to patients 65 years of age and older residing in the South Muskoka catchment area. The Seniors Assessment & Support Outreach (SASOT) is an interdisciplinary team that consists of two Social Workers, an Occupational Therapist, a Registered Nurse, a secretary and a physician medical advisor. Patients may be seen in their own home, a retirement residence, or in the emergency department at South Muskoka Memorial Hospital. SASOT completes a comprehensive assessment of a patient's health and day-to-day function, which may include:

| Public Comments | SASOT is an interdisciplinary team working to help maintain seniors within their homes, to facilitate successful hospital discharge, reduce emergency department visits and avoid unnecessary hospital admissions. SASOT will not duplicate services that already exist nor provide direct therapy such as psychotherapy, physical therapy, or wound care. |
