Description (Service) | A collaborative effort with community partners to provide visible, accessible, and reliable health promotion and healthy living information, education, and programs to the community. Programs and services include: Healthy Heart Program: Provides educational sessions to assist those with or at risk for heart disease to improve their lifestyle and prevent future illness. Eight-week mini-education session followed by exercise component that provides cardio and resistance training. Nurse Practitioner (NP): The NP provides general primary health care services for patients of all ages, and are involved in a variety of programs HELP (Healthy Everyday Lifestyle Program): A free program for adults who are at high risk of developing Type 2 diabetes and cardiovascular disease. Weight management through healthy eating and increased activity. Breastfeeding Support Clinic: Free Breastfeeding Support Clinic with lactation consultant held once a week.
