  • Home at Last - Support with discharge from hospital
  • Medical Equipment Program - accepts equipment donations

  • Blood pressure clinic

Hours Mon-Fri 10 am-1 pm
Blood Pressure Clinics: 3rd Tue of the month 10 am-11:30 am, no appointment required.

Eligibility 60 years and older
18 years and older with physical challenges
Valid Ontario Health Card required
Application Registration required for services. Referral by self or family member.
Languages English; Other - other languages may be available with assistance from volunteers
Fees Service - Some services are free, many services have a nominal fee ; No cost to register as a client
