Description (Service) Helps hospital patients successfully make the transition from hospital to home. Provides a Personal Support Worker to help clients "settle in" the day they are discharged from hospital. Services may include:
  • accompanying/driving a client home from hospital
  • picking up medications, medical equipment and/or groceries
  • making a small meal and doing light housekeeping
  • conducting a safety assessment
  • providing personal care
Operates as a partnership between Peterborough Regional Health Centre, Home and Community Care Support Services Central East, and Community Care Peterborough

Hours Referrals and inquiries: Mon-Fri 9 am-4 pm
Service: Mon-Fri by appointment
Areas Served Peterborough (County of)
Eligibility Adults who are frail or have special needs and may benefit from additional support settling in at home upon discharge from hospital 
18 year(s) and up;
Application Referrals may be made by any member of the client's care team.
