Description (Service)

Provides services that support health and wellbeing for seniors, adults with physical challenges and caregivers. Programs and services include:

Meals on Wheels - frozen entrées, soups, desserts
blood pressure clinic
Grocery Delivery
Transportation and accompaniment for medical appointments, shopping and errands, accessible transportation available via

Shopping buddy
Lifeline
Brokered Home Help and Maintenance
Friendly Visiting
Telephone Reassurance Checks
Exercise and Falls Prevention Classes - in-person and virtual options
Dining & Recreation Events
Home at Last - Support with discharge from hospital
Medical Equipment Program - accepts equipment donations

Hours	Mon-Fri 9:30 am-3 pm Blood Pressure Clinic: 2nd and 4th Thu of the month, 10 am-12 noon, no appointment required.
Areas Served	Curve Lake First Nation ; Selwyn Township ; Trent Lakes
Eligibility	60 years and older 18 years and older with physical challenges Valid Ontario Health Card required
Application	Registration required for services. Referral by self or family member.
Languages	English; Other - other languages may be available with assistance from volunteers
Fees	Service - some services are free, many services have a nominal fee ; No cost to register as a client
