Description (Service)

Provides services that support health and wellbeing for seniors, adults with physical challenges and caregivers. Programs and services include:

Meals on Wheels - frozen entrées, soups, desserts
Grocery Delivery
Transportation and accompaniment for medical appointments, shopping and errands, accessible transportation available via Caremobile
Shopping buddy
Lifeline
Brokered Home Help and Maintenance
Exercise and Falls Prevention Classes - in-person and virtual options
Friendly Visiting
Telephone Reassurance Checks
Dining & Recreation Events
Home at Last - Support with discharge from hospital
Medical Equipment Program - accepts equipment donations
Information and referral
Blood pressure clinic

This is an In and Out of Home Services

Hours	Mon-Thu 9:30 am-3:30 pm, Fri 9:30 am-12:30 pm Blood Pressure Clinic: 1st and 3rd Wed of the month, by appointment
Areas Served	Burleigh Falls ; Douro ; Selwyn Township ; Warsaw
Eligibility	60 years and older 18 years and older with physical challenges Valid Ontario Health Card required
Application	Registration required for services. Self referral (PET Healthy Aging/Housing) Blood Pressure Clinic: call to make appointment.
Languages	English; Other - other languages may be available with assistance from volunteers
Fees	Fees dependent on service (PET Healthy Aging/Housing) Some services are free, many services have a nominal fee ; No cost to register as a client
