Description

Description (Service) Services include:
  • supporting students to transition from school to adult life
  • assisting students to direct and develop an individualized transition plan in conjunction with people most important to the students
  • assisting students to explore and identify future community options available after high school, including post-secondary education, employment, volunteering, and social and recreational opportunities
  • securing and leveraging available resources, supports, and funding
  • liaising with the school system and other youth services and organizations
