Description (Service) | ILS assists adults with permanent physical disabilities and seniors with activities of daily living such as bathing, transferring, meal preparation, and household routines in two ways: PERSONAL SUPPORT SERVICES This service is prescheduled and available from 6 am to 12 am, 7 days a week as determined by the client's assessed needs. The level of service for this program is determined by an assessment. The service plan is developed with consideration of the client's preferences and available resources. Services are available at home, place of employment, or school. ASSISTED LIVING SERVICES Services are designed for clients who may require more frequent access to service on a 24-hour basis that cannot be met solely on a scheduled visitation model. Clients must live within a designated geographic service area. Both programs require: An annual service plan agreement which is developed with consideration of the client's preferences and available resources. In some cases, service levels will be assessed and reviewed as needed. Clients must be able to direct their care and have a backup plan in place. ............................................................................................................................... HOMEMAKING SERVICES Our Homemaking Program assists adults with physical disabilities and seniors with routine household activities such as light housekeeping (personal living space), household tasks, meal preparation, laundry, and shopping. Community referrals and care coordination is available. Service is available Monday to Friday from 8:00am - 5:00pm for a minimum of 1 hour visit, with a maximum of 4 hours per month. This is a fee-for-service program that is funded by Ontario Health with a subsidized rate based on the client's income. |
