Description (Service) | The Geriatric Assessment Intervention Network works team is for frail older adults living at home or in a retirement residence with multiple complex problems including one or more of the following:
Collaborates with the Central East Community Care Access Centre and Ross Memorial Hospital geriatric programs. Using best practices for seniors care, standardized assessments and protocols, individualized integrated care plans are created for high risk seniors. Clients are supported by case managers, who help clients navigate the health care system and access community programs. |
