Description (Service) | Support to individuals challenged by addiction issues to have the support they need to move towards a healthier lifestyle * Planning and delivery of culturally appropriate programs and activities, including social and recreational events * Referrals to external agencies where appropriate; ensuring cultural safety of as much as possible * Follow-up on referrals * Peer Support * Life skills development * Crisis support, assisting in arranging emergency assistance as may be required for access of service users to Detox, Medical Health Services and Treatment services * Advocacy * Referrals |
