Description (Service) Life Long Care Program supports Indigenous individuals from Birth to death. 
* Promotes improved quality of life and living for Indigenous people living with a disability, chronic illness, are frail elderly 
* Promotes holistic health and well-being in support of a healthy senior lifestyle 
* Information on local and regional health, wellness and long-term care support service agencies 
* Assists with some NIHB (Non-Insured Health Benefit) programs through Indigenous Services Canada
