Description (Service) | The Lifelong Care (LLC) Program provides cultural and community-based support services for urban Indigenous individuals of all ages who are living with a physical disability, chronic illness, or frailty due to aging. The program aims to promote independence, improve the quality of life, and encourage community participation through a holistic and integrated approach to care. By offering both practical and emotional support, LLC ensures individuals receive the assistance they need to live with dignity and autonomy in their homes and communities. Objective:

Eligibility | Urban Indigenous people of all ages who are living with a physical disability, chronic illness, or frailty due to aging.
