The Lifelong Care (LLC) Program provides cultural and community-based support services for urban Indigenous individuals of all ages who are living with a physical disability, chronic illness, or frailty due to aging. The program aims to promote independence, improve the quality of life, and encourage community participation through a holistic and integrated approach to care. By offering both practical and emotional support, LLC ensures individuals receive the assistance they need to live with dignity and autonomy in their homes and communities.

Objective:

  • Promote Independent Living: Support urban Indigenous individuals to live independently while maintaining a high quality of life.
  • Community Integration: Facilitate community participation and ensure that individuals feel connected to their cultural roots and community support systems.
  • Provide Holistic Care: Offer a range of services that address both physical and emotional well-being, particularly for those facing health challenges or aging-related issues.
  • Improve Access to Resources: Ensure individuals have access to the necessary services, including healthcare, housing, transportation, and cultural support.

Program Services:

  • Access to Traditional Medicines: Providing cultural healing practices alongside conventional medical care.
  • Transportation Support: Assistance with transportation for medical appointments, banking, and pharmacy visits.
  • Home Visits: Regular visits to assess needs and provide support in the home environment.
  • Health Care Assistance: Help with navigating health care systems, access to medical services, and overall well-being.
  • Social Dining: Providing opportunities for social meals to combat isolation and promote community engagement.
  • Recreational Opportunities: Encouraging active living through recreational activities to improve physical and mental health.
  • NIHB Navigation: Guidance and support in navigating the Non-Insured Health Benefits (NIHB) program, helping participants access health-related services and products that are not covered under other plans.
  • Security Checks: Regular security checks for individuals living alone or in vulnerable situations.
  • Indigenous Support: Providing culturally relevant support services to ensure a sense of belonging and connection to Indigenous heritage.

Outreach Services:

  • PSW Services: Personal Support Worker (PSW) services to assist with daily living tasks, personal care, and other essential needs.

Eligibility Urban Indigenous people of all ages who are living with a physical disability, chronic illness, or frailty due to aging. The program is open to individuals of all ages, with a particular focus on seniors, people with disabilities, and those with chronic health conditions. Open to those who live in urban areas and require support to maintain independence and improve their quality of life. No Restrictions on Background; The program is designed to support individuals regardless of their specific health conditions or circumstances.
