|Description (Service)||Urgent/emergent clinic provides comprehensive assessments, recommendations and treatment plans for high-risk seniors.
Inter-professional team includes nurse practitioners, physiotherapists, occupational therapists, social workers, pharmacists, Home and Community Care case managers and specialized physicians when required.
Seniors may be referred from the emergency department, other hospital services or their primary care providers. Patients generally: live at home or in a retirement residence * have multiple complex medical, functional and psychosocial problems * have recently experienced a functional or cognitive decline * have experienced falls or are at risk for falling.
To come prepared to their appointment seniors should bring: walking aids, eye glasses, hearing aids and all medications including over-the-counter products. Appointment may take two to three hours.
Also offers appointments at home for clients who have difficulty attending the clinic.
